Enrollment and Records

Freedom Academy holds open enrollment every year starting on February 1st. Applications are accepted on a “first-come, first-served” basis and are date-stamped in the order they are received. No applications will be deemed complete and date stamped until all documentation is received. Incomplete applications will not be considered for classes or wait lists. 

The Arizona Revised Statutes state that districts and charter schools may establish policies for the early admission of kindergarten and first grade students. Freedom Academy’s Board adopted policy is in alignment with the state recommendation that a child must reach the age of five (5) prior to September 1st to be eligible for kindergarten and must be age six (6) by September 1st to be eligible for the first grade. We make no exceptions to this policy.

Early advancement of high achieving students is not advised in most cases. Instead, the goal of Freedom Academy will be to individually enrich the program of a high functioning student instead of advancing them to a higher grade.


The following items must be filled out completely, signed and returned to the school office.

  • Enrollment Form
  • SPED Eligibility Form (located on the back of the Enrollment Form); If your child is eligible for special education services, their current IEP, MET (Psychological Evaluations is part of the MET) or 504 plan documentation.
  • Medical Waiver
  • Request for Records form
  • Original Birth Certificate - a copy will be made
  • Current and up-to-date Immunization Records
  • Arizona Residency Documentation Form
  • Copy of most recent report card
  • Copy of most recent standardized test scores (applying for grades 3-8)

Once a spot has been confirmed:

  • Completed placement exam - this test will be given to students applying to grades 1-8 to ensure they have mastered the academic skills necessary to enter the grade requested. Students who do not pass the test will not be admitted to the next grade.
  • Extracurricular Fee - this is a non-refundable fee
  • Packets are accepted throughout the week, Monday through Friday. Each Friday, completed packets are then considered for any spots available in the requested grade. You will receive a letter stating if your child has been accepted or placed on the wait list.

Freedom Academy will not enroll a student who is currently enrolled at another school without prior withdrawal forms. If Freedom Academy is notified of dual enrollment of a student, that secured spot is immediately forfeited.

Non Discrimination Policy

No person connected with Freedom Academy, Inc., whether a student, employee or volunteer shall, on the basis of gender, race, religion, or orientation be excluded from participation in, be denied the benefits of, or be subject to discrimination under any educational program or activity.


Bt state law (A.R.S. 15-83) your child will not be allowed to attend school until a record of immunization is submitted. Freedom Academy requires that all students be in compliance with immunization requirements. Medically verifiable, documented proof of immunizations must be provided prior to enrollment and attendance. Proof of immunization is defined as written documentation that includes the type of vaccine administered, the month and year of each immunization (except MMR immunization, for which the month, day and year is required), and the name of the physician or health agency administering the vaccine, or laboratory evidence of immunity.

Exemptions: If you wish for your child to be exempt from the immunization requirements, due to medical reasons, personal beliefs, or laboratory evidence, the “Request for Exemption to Immunization” form must be completed, signed and returned to the school. Please be aware that in the event of an outbreak of a vaccine preventable disease for which you cannot provide proof of immunity of your child, your child will not be allowed to attend school until the risk period ends.

Request for Records

Should you need a copy of your child’s records, you must submit a “Request for Records” form These forms are available in the front office. There is a 72 hour turn around for the records requested. We do provide one copy of these records for free, then a $15.00 fee will be charged for any additional copies requested. Please note that we do not charge to send records directly to another school.

Withdrawal Procedures

Withdrawal of your child from Freedom Academy requires a 72 hour notice to the front office staff prior to the student’s last day of attendance. This gives the office time to prepare your paperwork and the teacher time to gather your child’s class papers and grades. To withdraw a student, the parent or guardian must do the following:

Return any books and materials
Pay any outstanding fees
Obtain a Withdrawal Form. Without this form you will not be able to register at another Arizona school.
All fees paid to date are non-refundable.
IMPORTANT: Registration for new or returning students is contingent on current paperwork. If Freedom Academy receives requests for records for another school; the space at Freedom Academy is immediately forfeited.
