To enroll at either of our campuses, please download the enrollment packet to the right, or you can pick one up at either campus. Complete all forms in the enrollment packet and gather any required documents, and bring the completed forms to the campus of your choice. All required documents must be submitted. Incomplete enrollment packets will not be accepted.

Children are accepted to each grade level on a first come, first served basis. Each completed enrollment packet will be date stamped in the order in which they are received. If a grade is full, you may ask to be placed on the waiting list.

All enrollment packet forms
Original birth certificate
Immunization records
Withdrawal form from previous school
Proof of Arizona residency (drivers license, utility bill, etc.)
Copy of the most recent report card
Copy of the most recent standardized test scores (for grades 3-8)
